Adult clients need to complete the Adult Intake Forms and bring them to the first appointment, and should download the Disclosure/Privacy Rights form.
Parents should complete the Child Intake Forms for the child and bring them to the first appointment. Parents should download the Disclosure/Privacy Rights form. Complete Release of Information if sharing of information is needed.
